1. Introduction
Midtown Psychology Legal, a division of Sharpe Social Work Professional Corporation, is committed to protecting your privacy and personal information. We comply with the Personal Health Information Protection Act (PHIPA), the Personal Information Protection and Electronic Documents Act (PIPEDA), and the Standards of Practice of the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
2. Collection and Use of Information
We collect personal, health, and contact information for the purpose of providing psychotherapy and related professional services. This includes your name, contact details, health history, session notes, and payment information.< /p>
We use this information to:
- Deliver psychotherapy and related professional services;
- Communicate with you, your lawyer, or your other health-care providers (with your consent);
- Maintain required clinical records; and
- Process payments and issue insurance-eligible receipts.
3. Storage and Security of Information
All client records are securely stored using Jane App, a PHIPA-compliant electronic health record platform. Access is restricted to authorized clinicians and administrative staff.
We take reasonable precautions to prevent unauthorized access, loss, or misuse of information. Electronic communications such as email and fax are used only when appropriate safeguards are in place. While we take all reasonable steps to protect your information, the use of electronic communication carries minimal inherent risks.4. Communication With Other Professionals
As part of providing coordinated care, we may communicate with your lawyer, physician, psychiatrist, or other treatment providers when necessary and with your consent. Such communication is limited to information relevant to your treatment goals or legal context.
Letters and reports (including Letters of Completion) are sent by fax or email only after full payment has been received.
5. Payment and Financial Information
Payment may be made by credit card or e-transfer. Receipts are provided for insurance reimbursement. Personal and financial information is handled in compliance with PHIPA and PIPEDA security standards.
6. Website Data Collection
Our website collects standard data such as browser type, device information, and usage statistics through cookies or analytics tools. This information is used to improve our website experience and is not linked to your health record.
7. Limits to Confidentiality and Legal Exceptions
We are legally required to disclose information without consent in specific circumstances, including:
- When there is a risk of serious harm to self or others;
- When there is reason to believe that a child may be in need of protection;
- When records are subpoenaed by a court; or
- When disclosure is otherwise required by law.
8. Retention and Access to Records
Clinical records are retained for at least 10 years after the last client contact (or 10 years after the client turns 18, whichever is longer), in accordance with OCSWSSW standards. You have the right to access or request corrections to your personal health information by contacting our Privacy Officer.
